Begin your journey to independence with GUIA by taking the first step – simply fill out our convenient online referral form. It’s quick and easy, taking just 5 minutes of your time. Once submitted, our dedicated team at our head office will promptly start the process, ensuring you receive the personalised support and care you need to thrive.

After submitting your online referral form, our team will reach out to introduce ourselves, gather more information about your needs, and schedule a convenient meet-and-greet.
This allows us to get to know you better and ensure we’re prepared to provide the most personalised and effective support.

At the meet-and-greet, we will present suitable staff profiles, work with you to create a personalised support plan, complete any necessary paperwork, and confirm your support schedule.
This process typically takes about two hours, allowing us to establish a tailored plan that meets your specific needs and preferences.

After your initial support has been provided, we’ll check in regularly. We will call to ensure your service satisfaction, keeping your support coordinator and/or family informed along the way.
